Our Referral Program

As we all know, there are always a few rules we have to abide by:

• Only the individual who refers someone to our practice is eligible to receive the gift card. The referred patient will not receive a gift card.
• The referral program is only open to clients of Dr Farrugia at BodyLuxe who have already had a surgical procedure at BodyLuxe or have their surgery scheduled.
• The referred client must list you as the person who referred them when completing the online enquiry form. Once the consultation request form is submitted, we will not honor requests to add a referral.
• The surgical procedure must occur within 6 months from the time of submitting the enquiry form. No gift card will be awarded if the procedure takes place more than 6 months after the consultation request form was filled mentioning the referral source.
• Only the initial consultation request form with the original referral source is considered for the purpose of awarding a gift card. Filling in a new consultation request form with a different referral source or a different date will not be valid for participation in the referral program.
• We will not honor retroactive requests, i.e. we will not credit referrals for past procedures.
• The gift card will only be awarded to one person, which is the one listed as the referral source on the initial consultation request form. We will not issue split gift cards if the referral source is more than one person. If there is a question regarding who is the referral source, the client’s preference for awarding the gift card will be strongly considered. 
• Only one referral per household is eligible for a gift card; e.g. only one spouse of a married couple counts as a referral. Additionally, members of the same household will not receive a gift card for referring another member of the household.
• Gift cards are non-transferable, and may not be redeemed for cash at our office.
• The procedure must be a minimum of $5,000 to qualify for the gift card.
• Your gift card will be sent to you by regular postal mail to your address on file within 45 days after the referred client completes their procedure. You can also pick up the gift card at our office.
• We are not responsible for lost or stolen gift cards, or cards that did not arrive in the mail. 
• We will not replace gift cards under any circumstance. 
• Gift cards cannot be applied as a payment on an outstanding balance or previous surgery or service.
• Physicians and physician offices are excluded from the Patient Referral Program due to regulations.
